BACK OVER PREVENTION
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."
Consequence
A rearview camera that fails to display an image can reduce the driver's view behind the vehicle, increasing the risk of a crash.
Remedy
Dealers will update the parking assist software, free of charge. Owner letters were mailed January 2, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB13 and 25LB06. Subaru's number for this recall is WRE25.
ELECTRICAL SYSTEM
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2023-2024 Toyota Venza, 2023-2025 RAV4 Prime, RAV4, Highlander, GR Corolla, Crown, 2024-2025 Lexus TX, LS, Toyota Tacoma, Grand Highlander, and 2025 Lexus RX, Toyota Crown Signia, Camry, RAV 4 Plug-in Hybrid (PHEV), and 4 Runner vehicles. Due to an error in the instrument panel software at vehicle startup, the instrument panel may fail to display vehicle speed, brake system, and tire pressure warning lights.
Consequence
An instrument panel display that does not show critical information can increase the risk of a crash or injury.
Remedy
Dealers will update the instrument panel software for non-PHEV vehicles, free of charge. For PHEV vehicles, dealers will inspect the instrument panel assembly, and either replace it, or update the software, free of charge. Owner letters were mailed December 5, 2025. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB08 and 25TA08. Lexus' numbers for this recall are 25LB05 and 25LA05.
SEAT BELTS
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2025 Toyota Camry Hybrid, Lexus NX250, NX350, NX350 Hybrid, 2024-2025 Lexus RX350, RX350 Hybrid, and RX500 Hybrid vehicles. The second-row center seat belt webbing may be damaged. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 209, "Seat Belt Assemblies."
Consequence
A damaged seat belt may not adequately restrain an occupant, increasing the risk of injury during a crash.
Remedy
Dealers will inspect the seat belt webbing and replace the seat belt assembly if necessary, free of charge. Owner notification letters were mailed March 20, 2025. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TA03 and 25LA03.

1 complaintI was driving the vehicle up a road I drive every day, when suddenly the instrument cluster showed a picture of a red pedestrian icon, and slammed on the breaks. It threw me forward, but quickly proceeded on like normal. It detected a pedestrian that was not in front of the vehicle. This was only 20 days after I got the car. It proceeded to do this three more times, with similar severity, but at different speeds and different locations. The fourth instance, I was driving home and the vehicle had the same pedestrian detection of an imaginary person, this time breaking me from 40 mph to 12 mph with a car behind me. Thank the lord I was not hit. I made a service appointment asap, and it was there for 8 days. I had it happen to me 5 times in 3300 miles and 2.5 months, and they drove it 12 miles and said they couldn’t duplicate it. Cooperate told me to send it up to another store which I did. It has now been there for 16 days, and they can’t find an issue either. I am not comfortable driving the car because it could have a breaking incident at any time, and is putting me in danger. Cooperate has told me to file a lemon case or quote “wait for it to happen again and try and document it in better detail.” I have given them locations, time and date estimates and weather estimates for every instance, and I’m still getting put back in a car that someone at Hendrick Lexus North considered a “liability.” I am beyond disappointed in Lexus for going against their word, is making me the guinea pig in a effort to try to replicate the issue. This is my third Lexus, and my image of the brand is ruined.

Also, the manufacturer's APP indicated a malfunction in the engine and engine throttle during the incident. The driver sustained injuries to the knees, arms, and hips, which required medical treatment. An incident report was filed by the facility Patrol Officer, and the vehicle was towed to the collision center. The cause of the failures was not determined. The manufacturer and local dealer were notified of the incident. An engineer from the manufacturer was sent to examine the vehicle and reported no trouble found. The contact indicated that upon delivery of the vehicle, several electronic features did not function properly, including the display and navigation screen. The manufacturer and local dealer were notified of the failure. The failure mileage was 106.

1 complaintI bought my car on May 30, 2025, and on June 3, 2025 while driving, the driver’s side rear window behind the passenger’s seat shattered. It sounded like an explosion. No cars were around at the time of the incident. Luckily, the window stayed intact.

The contact stated that there was a loud sound coming from the engine compartment. There was no warning light illuminated. The contact forcefully depressed the brake pedal to stop the vehicle. The vehicle was towed to the dealer but was not diagnosed or repaired. The manufacturer was made aware of the failure. The approximate failure mileage was 119.

1 complaint1) Electrical malfunction with warning of locking malfunction transiently appearing 2) Locked inside the car and the manual door release would not work after the above. 3) Finally got out after using key fob and multiple attempts to use the door handle, closed the door, and the car locked and could not open it with either the key fob, Lexus application, nor with the emergency physical key (it would turn but not unlock the door). 4) After 1st malfunction, had a smell of burned electrical component but NO smoke or fire. The hybrid system was fully charged however, it is probable that after speaking to Lexus while inside the car with the ignition on to trouble shoot a charging scheduling issue, the 12 volt battery was partially discharged. The car has 441 miles and after Lexus service arrived, they were able to get it drivable HOWEVER, check engine light remained on and "system malfunction...visit your dealer" was also on the entire 7 mile trip to the dealer with the tow truck following me all the way. I kept the window open despite the rain so that I could get out if something else happened.
